From this page you can access the National Mozart Competition Entry Form.

You have two options here:

Print it out and fill it out in pen, sending it to the address given at the bottom of the form;

Fill it out on the computer and then print out the completed form before sending it off (don't forget to sign it!)

Either way, to make full use of all the helpful features contained within the form, you will need Adobe Acrobat Reader, version 5.0 or above.

To download this free software, from the manufacturers, click on the icon below.

Find "Acrobat Reader" and click select your computer's platform (usually Windows or Mac)

On the next page, select "Acrobat Reader for Windows - All languages" and then follow the simple instructions that appear.
